APPLICATION GUIDELINES

• Children and young adults 18 years of age or younger who are have a diagnosis of a brain tumor, or are currently being treated for a brain tumor are eligible to apply.

• Applications must be completed in full and be signed off on by a medical provider, hospital social worker or designated hospital representative.

• Applications must be received by the 15th of each month to be considered during that month’s review.

• Applications are reviewed once per month.

• Approved payments will be issued by the 1st of the following month, when possible.

• The Foundation makes payments directly to third-party service providers whenever applicable.

• Copies of all bills or invoices must accompany the application.

• Bills must clearly display the provider's name, mailing address, account number, and amount due.

• The Foundation determines assistance based on the individual circumstances of each family, the information provided, and available Foundation resources.

• Submission of an application does not guarantee financial assistance.

Examples of Expenses That May Be Covered

Alessia's Gifts may include, but not limited to:

• Special family outings or experiences

• Birthday or holiday celebrations

• Tickets to special events

• Family activities or day trips

• Hobbies, toys, or special items. Allowed for siblings as well

• A child's personal dream or special request

• Meaningful experiences designed to create lasting family memories

Financial Assistance may include, but is not limited to:

• Rent or mortgage payments

• Lodging associated with medical treatment

• Transportation or travel expenses related to treatment

• Car payments

• Utilities

• Essential household expenses

• Other emergency or treatment-related expenses approved by the Foundation

CONFIDENTIALITY

All information submitted to Alessia Maria’s Light of Love Foundation will be treated with care and kept confidential.

Information provided through this application will be used solely for the purpose of reviewing and administering assistance through the Light of Love Programs, except where disclosure is required by law or separately authorized.

A photo or media release is NOT required to receive financial assistance.

Families who wish to share their story, photographs, or their child's journey to help further Alessia's legacy may choose to complete a separate, voluntary Photo & Story Release.
